基于CiteSpace的中药治疗心力衰竭中英文研究热点与趋势的对比分析(2006—2025)

摘要:

目的旨在运用可视化技术,系统梳理并对比2006—2025年中药治疗心力衰竭(HF)的中英文文献,旨在解析该领域的研究现状。方法检索中国知网(CNKI)、万方(Wanfang)、维普(VIP)获取中文文献,检索WebofScience(WOS)核心合集获取英文文献。利用CiteSpace6.5.R6软件对发文量、作者、机构、国家及关键词进行可视化分析。结果研究最终纳入中文文献688篇,英文文献829篇。发文趋势显示,2016年后英文高质量文献产出增长迅速,显示国际关注度提升。中文研究形成了以张艳、朱明军等为核心的稳定团队,侧重临床经验传承与用药规律;英文研究以WangWei、WangYong等团队为代表,聚焦前沿机制。机构分析显示,北京中医药大学与中国中医科学院在国际学术影响力上占据显著优势。关键词分析显示中英文研究均关注“心肌梗死”与“炎症反应”。中文研究侧重于“临床经验总结”与“方法学创新”;英文研究则呈现“微观化与分子化”趋势,聚焦于氧化应激、细胞凋亡、自噬及特定信号通路。铁死亡、肠道菌群、分子对接以及基于系统生物学的机制阐释是当前最活跃的研究前沿。结论中药治疗心衰的研究已从宏观临床观察深入到微观分子层面,核心机制聚焦于利用中药复方及活性单体(如黄芪多糖等)通过SLC7A11/GPX4抑制铁死亡、阻断TLR4/NF-κB炎症通路、改善线粒体能量代谢及遏制心肌细胞凋亡。随着老年患者多重用药日益普遍,中西药联用产生的潜在不适当用药(PIM)风险凸显。当前国际主流PIM标准缺失中药条目,而国内标准滞后于现代药理学研究。未来应在深挖“多靶点”机制的同时,亟需构建涵盖现代中西药相互作用的安全预警体系。

Abstract:

Objective To systematically review and compare Chinese and English literature on traditional Chinese medicine (TCM) for heart failure (HF) from 2006 to 2025 using visualization techniques, thereby elucidating the current research landscape. Methods Chinese literature was retrieved from CNKI, Wanfang and VIP databases, while English literature was sourced from the Web of Science (WoS) Core Collection. CiteSpace 6.5.R6 software was utilized to conduct a visual analysis of publication volume, authors, institutions, countries and keywords. Results Total of 688 Chinese and 829 English publications were ultimately included. Publication trends indicate a rapid increase in high-quality English output since 2016, reflecting growing international attention. Chinese research is characterized by established groups—primarily led by Zhang Yan and Zhu Mingjun —focusing on documenting clinical experience and prescription patterns. Conversely, English research, represented by Wang Wei and Wang Yong, primarily investigates cutting-edge mechanisms. Institutional analysis reveals that the Beijing University of Chinese Medicine and the China Academy of Chinese Medical Sciences hold a significant advantage in international academic influence. Keyword analysis demonstrates that both Chinese and English studies emphasize "myocardial infarction" and "inflammatory responses." Chinese studies lean towards "clinical experience summaries" and "methodological innovations", whereas English studies exhibit a "microscopic and molecular" trend, focusing on oxidative stress, apoptosis, autophagy and specific signaling pathways. Ferroptosis, gut microbiota, molecular docking and systems biology-based mechanistic elucidations represent the most active current research frontiers. Conclusion Research on TCM for HF has progressed from macroscopic clinical observations to microscopic molecular levels. The core mechanisms primarily involve utilizing TCM formulas and active monomers (e.g., astragalus polysaccharides) to inhibit ferroptosis via the SLC7A11/GPX4 axis, block the TLR4/NF-κB inflammatory pathway, improve mitochondrial energy metabolism, and suppress cardiomyocyte apoptosis. With the increasing prevalence of polypharmacy among elderly patients, the risk of potentially inappropriate medications (PIMs) arising from the combined use of Chinese and Western medicines has become prominent. Current mainstream international PIM criteria lack TCM-specific items, while domestic standards lag behind modern pharmacological research. Future research should not only delve deeper into the "multi-target" mechanisms of TCM but also urgently construct a safety warning system encompassing modern Chinese-Western drug interactions.
